    Application for Admission  Use this form to apply for admission to Northeast State.
    Hepatitis B Immunization Health History  Use this form if you will be attending Northeast State for the first time. The State of Tennessee requires that all students attending Northeast State for the first time be provided with information concerning Hepatitis B infection. Please read this form carefully, complete the required section, sign it, and return it to the Northeast State Office of Admissions and Records.This form must be on file before attending classes. 
    Application for Readmission  Use this form if you are a returning student and have not attended Northeast State for two consecutive semesters.
    Class Registration Form  Use this form to register for courses you plan on taking at Northeast State.
    Request for Transcript  Use this form to have your official high school, college, or GED transcript sent to Northeast State. NOTE: This form should be completed and mailed to your high school, college, or GED test center, NOT to Northeast State.
    Request for Northeast State Transcript  Use this form to request that your official Northeast State transcript be sent to you or a third party.
    Request for Third Party Release  Use this form to authorize Northeast State to release information to a third party such as an employer or parent.
    Request for Change of Major/Catalog  Use this form to request a change in your major or catalog.
    Request for Name/Address Change  Use this form to request a name, telephone and/or address change.
    Request to Prevent Disclosure of Directory Information  Use this form to withhold the release of your directory information.
    Request to Drop/Add Courses  Use this form to drop or add courses.
    Request to Repeat a Course  Use this form to request approval to repeat a grade of A or B.
    Request to Withdraw  Use this form to withdraw from all your classes.
    Web Application Signature Page  Use this form if you have submitted an online application for admission and were unable to print the signature page.
