How do I register with the Center for Students with Disabilities (CSD)?
  • Contact the CSD office by phone or in person.
  • Provide written documentation of the disability. (Documentation guidelines are available upon request.)
  • The Coordinator will review the documentation and inform the student if additional information is required.
  • The student schedules a Needs Assessment with Coordinator of the CSD.
  • The Coordinator will determine with the student appropriate accommodations and support services to be provided in the classroom.

What kind of documentation is required for me to receive accommodations? 

  • A recent IEP from your high school with current test scores – must be no older than 3 years.
  • A recent, ongoing, or chronic diagnosis from your physician, psychologist, or other qualified and licensed mental health professional.  (Forms are available in the CSD office and on the Northeast State website that you can take to your health professional to be completed).

Do I have to bring new updated documentation every year? No, unless you have a new diagnosis which will result in the need for more or different accommodations.

What kinds of accommodations can I receive from the CSD? 

  • Accommodations include, but are not limited to:
  • Extended time on tests
  • Alternative testing location
  • Note-takers
  • Scribes for tests
  • Interpreters for deaf/hard of hearing
  • Alternate seating arrangements
  • Tape recording of class lectures
  • Adaptive software
  • Books on PDF

How do I receive accommodation letters and get them to my instructors?
 

  • Within two days after your Needs Assessment, your letter will be ready to pick up from the CSD office.
  • You will meet with the Coordinator to review and sign your letter.
  • A CSD staff member will e-mail your letter to your stumail (Northeast State Student Email) and show you how to forward the letter on to your instructors.

Do I have to sign up for my accommodation letters every semester? Yes, at the beginning of each semester you should come by the CSD office and sign up for letters. If you are unable to come to the main campus, you can call the CSD office and request your letters.

What is Vocational Rehabilitation (VR or Voc Rehab)? 

  • Vocational Rehabilitation is a program of the Tennessee Department of Human Services - Division of Rehabilitation Services- which works to enhance the development of a diverse workforce.
  • Individuals who are eligible:
  • Have a physical or mental disability that constitutes or results in a substantial impediment to employment.
  • Can benefit from VR services to reach an employment outcome.
  • Require VR services to prepare for, obtain, keep or regain employment.
