Consortium Agreement Request

This form is used as a contract between two colleges/universities that recognizes the registration of a student at each site for financial aid purposes. It also certifies that only one of the two colleges/universities will administer Title IV financial aid for the student. Click here to learn more about the Consortium Agreement. Deferment of Maintenance Fees
| This form is no longer available for download but can be obtained in the Financial Aid Office located in the Faculty Building, Room 102. This form must be completed when a student expects to use loan monies to pay his/her tuition/fees. A Stafford Loan Information Worksheet must be completed and submitted to the Financial Aid Office before a deferment can be accepted. The deferment will only hold tuition/fees until the loan proceeds are received by the Financial Aid Office. IT IS THE STUDENT'S RESPONSIBILITY TO REQUEST THIS DEFERMENT. This form is to be completed by the student and the parent (if student is dependent). The, Northeast State, Direct Loan Information Worksheet is an interactive form that can be completed online, printed, and submitted to the Financial Aid Office: F-102; the worksheet must be submitted before the loan can be processed. A loan will not be certified until the student has completed the Free Application For Student Aid (FAFSA)and completed the Entrance Online Counseling. Once a loan has been certified and the student has received an award letter, a signedMaster Promissory Note (MPN) must be completed and submitted electronically to the Department of Education at www.StudentLoans.gov before funds can be released to Northeast State.

This form is to be completed if a student is interested in the Federal Work Study Program (FWS). To participate in the FWS program the student must complete and submit a FAFSA and have eligibility for FWS funding.
FWS timesheets are available under campus forms on Polaris. This form is used to log time worked by students under the FWS program. |

This form is used because you indicated, on the FAFSA, that you would have a master’s or doctorate degree, or would be receiving a bachelor's degree or equivalent. The Financial Aid Office will use it to determine the validity of your answer and to determine if you are eligible for Federal Pell funds. You must print, check the box that applies to you, sign, and return it to the Financial Aid Office F-102. For faster results return to FAFSA website and make the necessary correction as well. |

You can order your IRS Tax Transcript online by clicking here Order IRS Transcript and completing the online application. Or you by completing the Short Form Request for Individual Tax Return Transcripts, (Form 4506T-EZ) located in the left-hand column. This will help taxpayers get their return information quickly and easily. The transcript can also be ordered using the IRS self-service line at 1-800-908-9946. If you order form(s) by phone, follow the prompts and select 3, then 1 to quickly complete your order. Once you get your transcript order form, complete it, sign it and send it to the address listed in the instructions. Either way, you can expect to get your transcript within 5 to 10 days from the time the IRS office receives your signed request. |

Supporting documentation is required and must be submitted to the Financial Aid Office along with the completed FAFSA. Due to the Family Educational Rights and Privacy Act of 1974 our office is not permitted to discuss your Financial Aid Account with anyone other than yourself. If you would like to give the Financial Aid Office permission to speak with someone other than yourself about your account you must complete and return this form to the Financial Aid Office. This form only permits the designated person to inquire about your Financial Aid and is not recognized by other offices at Northeast State Community College. This form must be signed by the student in the presence of a financial aid representative. |
