Federal regulations require total amounts of cash, savings, and checking; net value of business (if less than 100 full-time employees) and/or farm (do not include the home you live in); and net value of investment (stocks, bonds, CD’s, real estate, etc.), be reported as of the date the FAFSA application was completed.

    This form is used as a contract between two colleges/universities that recognizes the registration of a student at each site for financial aid purposes. It also certifies that only one of the two colleges/universities will administer Title IV financial aid for the student. Click here to learn more about the Consortium Agreement.     This form must be completed when a student has answered "Yes" or did not answer question 23 on the 2011-2012 or 2012-2013 FAFSA's.

    Federal Work Study Program 

    Federal Work Study Program  This form is to be completed if a student is interested in the Federal Work Study Program (FWS). To participate in the FWS program the student must complete and submit a FAFSA and have eligibility for FWS funding.

    FWS timesheets are available under campus forms on Polaris. This form is used to log time worked by students under the FWS program.

    This form is used because you indicated, on the FAFSA, that you would have a master’s or doctorate degree, or would be receiving a bachelor's degree or equivalent. The Financial Aid Office will use it to determine the validity of your answer and to determine if you are eligible for Federal Pell funds. You must print, check the box that applies to you, sign, and return it to the Financial Aid Office F-102. For faster results return to FAFSA website and make the necessary correction as well.

    You can order your IRS Tax Transcript online by clicking here Order IRS Transcript and completing the online application. Or you by completing the Short Form Request for Individual Tax Return Transcripts, (Form 4506T-EZ) located in the left-hand column. This will help taxpayers get their return information quickly and easily. The transcript can also be ordered using the IRS self-service line at 1-800-908-9946. If you order form(s) by phone, follow the prompts and select 3, then 1 to quickly complete your order. Once you get your transcript order form, complete it, sign it and send it to the address listed in the instructions. Either way, you can expect to get your transcript within 5 to 10 days from the time the IRS office receives your signed request.

    Northeast State Online General Scholarship Application 

    Northeast State Online General Scholarship Application  This is the application for scholarships offered through Northeast State Community College. The application must be submitted by March 1st of each academic year. For more details visit Northeast State Scholarships.

    Petition for Dependancy Override 

    Petition for Dependancy Override  This form is used to request an override of a student's Dependency Status as defined by the U.S. Department of Education. Supporting documentation is required and must be submitted to the Financial Aid Office along with the completed FAFSA. Contact the Financial Aid Office at 423-323-0252 if you have already completed the FAFSA and would like to request a Dependency Override.

    This form is to be completed only if a significant change in the student's family circumstances occurs. For example, loss of student's/parent's/spouse's income, separation or divorce of student/parent, or death of spouse/parent. Supporting documentation is required.

    Satisfactory Academic Progress (SAP) Appeal

    Third Party Release Form 

    Third Party Release Form  Due to the Family Educational Rights and Privacy Act of 1974 our office is not permitted to discuss your Financial Aid Account with anyone other than yourself. If you would like to give the Financial Aid Office permission to speak with someone other than yourself about your account you must complete and return this form to the Financial Aid Office. This form only permits the designated person to inquire about your Financial Aid and is not recognized by other offices at Northeast State Community College. This form must be signed by the student in the presence of a financial aid representative.
