Documentation of Disabilities

Documentation from a licensed or certified professional is an important component in developing an accommodation plan. Students with a medical or psychological disability must provide current documentation (within the past three years.) Students with ADD/ADHD or a learning disability must provide a psychoeducational assessment. Student's who utilized an Individualized Education Plan (IEP) or 504 Plan in high school, should provide a copy of their last plan as part of their documentation packet.

All documentation should state how the student's disability will impact their success in college and list recommended appropriate accommodations.
