Northeast State is committed to providing equal access to programs, services, and activities for students with disabilities in accordance with the Americans with Disabilities Act (ADA), Section 504 of the Rehabilitation Act, and applicable state law. To determine eligibility for accommodations, Accessibility Services requires appropriate documentation of a disability.

What your documentation should include:

  • Written by a licensed professional (doctor, psychologist, counselor, etc.).
  • Their name, title, credentials, and date of evaluation.
  • A diagnosis of your disability and when it was identified.
  • A description of how your disability affects you in school or daily life.
  • Any tests, medical records, or evaluations that support the diagnosis.
  • Suggested accommodations (we will review these, but final accommodations are decided by the Accessibility Services team).
What you need to do:
  • Turn in your documentation early so we have time to review it.
  • Meet with Accessibility Services to talk about how your disability affects you and what supports will help.
  • Know that your information is confidential—we only use it to set up your accommodations.

Accessibility Services reserves the right to request additional information if documentation is incomplete, unclear, or does not adequately support the need for accommodations.
